If, however, you prefer to fax or email your forms, simply print the form, fill it out, sign it (if you are donating via payroll deduction), and send it to Paula Enderle, UConn-Storrs coordinator, U-5075. You can also fax it to 860-486-0379, or scan it and send to csec@uconn.edu. If you are making a one-time gift, please don’t forget to enclose your check payable to the Connecticut State Employees’ Campaign (or CSEC).
