As a reminder, the research team is not able to see the actual responses you provide and those responses will NOT be connected to your contact information.
You can complete online via a secure website and your participation will be completely anonymous. Only the aggregated group results will be published in a report. No individual responses will be reported. The survey results will be made available to all state employees. The information collected in this survey will not be used to satisfy the University’s federal reporting obligations.
