2014 Health Enhancement Program (HEP) News

The deadline to complete your 2014 HEP requirements – by Dec. 31, 2014 – is approaching!

Check your status online at www.cthep.com – and have dependents do the same – to verify whether anyone in the household has any outstanding requirements.

To continue receiving the financial advantages of HEP, please remember that every individual in the household must meet all requirements for both preventive and chronic condition(s), if applicable.

    NEW:  HEP Clinics

We are pleased to announce more than 35 HEP clinics scheduled at worksites throughout the state this fall! HEP representatives will be available to help you:

    • Check your HEP status
    • Complete chronic condition(s) requirements quickly and easily on site
    • Answer any questions you have about the program, your status and navigating the HEP portal.

    What is a preventive exam?

Many HEP participants are due for a “preventive exam.” This is your routine physical, usually with your primary care provider, and should include lab work, particularly a cholesterol screening.

Be sure to tell your physician that you want a full routine physical and cholesterol screening, and you do NOT need to wait 365 days from last year’s physical for it to be covered.

    Schedule appointments now!

Visit the HEP portal – www.cthep.com – and schedule any outstanding appointments as soon as possible to avoid the late-year rush to the doctor or dentist.

For questions, please contact Care Management Solutions toll free at (877) 687-1448.  HEP representatives will be available Monday through Thursday from 8 a.m. to 6 p.m. and Friday from 8 a.m. to 5 p.m.
