    Important Notice 

     Health Enhancement Program (HEP) Compliance and Exemptions 

    Last December Care Management Solutions (CMS) sent you and your dependents a personalized letter either confirming your HEP compliance or identifying your outstanding preventive and chronic requirements if applicable. 

    Employees and any dependents enrolled in the Health Enhancement Program (HEP) must complete all required HEP preventive exams and screenings by February 28, 2014.

    Employees can request an exemption from the HEP requirements for:

          1.  dependents currently serving in active military duty

          2.  non-custodial children

          3.  for themselves and/or their dependents based on adherence to religious beliefs.

    To request the exemption, please go to the OSC website at    http://www.osc.ct.gov/benefits/HEPforms.htm and complete the applicable non-compliance exemption form. 

    • CO-1324 – Military Service Exemption Form Non-Compliance Response
    • CO-1325 – Non-Custodial Parent Exemption Form Non-Compliance Response
    • CO-1326 - Religious Exemption Non-Compliance Response

    Completed exemption forms must be submitted to the OSC Healthcare Policy & Benefit Services Division for review.
