The Office of Logistics Mangement (OLM) no longer accepts paper forms for Capital and Controllable equipment. Effective July 1st, 2017 OLM only processes LM-1, LM-2, IC-5 and IC-6 requests using the online Electronic Forms Workflow (EFW) system.
