All applications and checks should be payable to BJ’s Wholesale Club, Inc. and should be mailed to the Office of the The State Comptroller, 55 Elm Street, Hartford, CT 06106 to the attention of Jacqueline Kozin. If paying by check a driver’s license number is required. Please do not add sales tax. This special offer is not valid online or at any BJ’s Club locations. A temporary shopping pass is sent via email after receiving your application and check. On a separate sheet of paper, please include your current state agency and email address where you would like a confirmation sent. Please be sure your family member, friend or neighbor indicate the state agency as well so credit goes to the appropriate agency. Renewals will not receive a new card, but can check with BJ’s for the status of their renewal application.
