Do you use opiates for recreational or nonmedical purposes? Examples may include:
Percocet ("Perc"), Oxycodone ("Oxy"), Vicodin ("Vikes"), Hydrocodone, Dilaudid, or other presciption pain pills
Codeine Syrup ("Lean")
Other opiates such as Morphine, Heroin, or Fentanyl
