2014 Health Enhancement Program: FIVE WEEKS LEFT!
The deadline to complete 2014 Health Enhancement Program (HEP) requirements – by Dec. 31, 2014 – is approaching if you or any dependents haven’t already done so.
NEW: We have heard from several physician groups – with locations in every region in Connecticut – who say they can accommodate HEP participants who still need a HEP preventive exam (commonly known as a standard physical exam) before the deadline.
If you or anyone in your household is having trouble scheduling an outstanding physical/preventive exam with your doctor before the end of the year, click this link for a list of physicians that say they can accommodate you:

Check your status
If you’re unsure about your status, you can check your status online at www.cthep.com at any time. Be sure to have dependents do the same.
Please remember that to continue receiving the financial advantages of HEP, every individual in the household must meet all requirements for both preventive and chronic condition(s), if applicable.
What is a preventive exam?
Many HEP participants are due for a “preventive exam.” This is your routine physical, usually with your primary care provider, and should include lab work, particularly a cholesterol screening.
Be sure to tell your physician that you want a full routine physical and cholesterol screening, and you do NOT need to wait 365 days from last year’s physical for it to be covered.
Don’t forget your dental!
This is a reminder that HEP participants who are on the state dental plan must complete at least one dental cleaning per year. Your dental compliance should be reflected in the online HEP portal, so please check your status at www.cthep.com to verify whether you’ve already completed your dental requirement this year.
For questions, please contact "Care Management Solutions at (877) 687-1448. HEP representatives will be available Monday through Thursday from 8 a.m. to 6 p.m. and Friday from 8 a.m. to 5 p.m.
