  • UCPEA Performance Evaluation Form

    We write to share that the University has transitioned the UCPEA performance evaluation form to Kuali Build, which is available under UCPEA on the HR Form’s page: https://hr.uconn.edu/hr-forms/.

    For supervisors, please find guidance below on navigating Kuali Build to complete the evaluation.

     The evaluation process follows this workflow:

    1. Supervisor completes the evaluation.
      1. Supervisor has the option to send a preview of the evaluation the first supervisor outside of UCPEA. The supervisor outside of UCPEA cannot modify the evaluation.
      2. Supervisor has the option to send the evaluation to Labor Relations for review. Labor Relations may modify the evaluation in collaboration with the supervisor.
      3. For evaluations with an overall rating of Unsatisfactory or In Needs of Improvement, the evaluation will route to Labor Relations automatically. Labor Relations may modify the evaluation in collaboration with the supervisor.
    2. The supervisor will receive two automated emails: 1. A copy of the evaluation in PDF and 2. Instructions on returning to the evaluation after the one-on-one employee meeting.
    3. Supervisor sends PDF of evaluation to employee prior to the one-on-one meeting, and the supervisor and employee then meet one-on-one either virtually or in person.
    4. Supervisor makes any discussed and agreed upon modifications to the evaluation, enters the date of the one-on-one meeting, and digitally signs the evaluation. The evaluation will route to the employee.
    5. Employee adds optional comments to the evaluation and digitally signs.
    6. The employee, supervisor, and HR receive PDF copies of the completed evaluation.
    7. In addition to receiving a PDF copy of the completed evaluation, the employe will receive a link to an additional Kuali Build form, where the employee may enter or upload amended comments to the evaluation record.
