2014 Health Enhancement Program (HEP)
Spring has arrived – the perfect time for you and your enrolled dependents (if applicable) to plan ahead for your 2014 HEP requirements.
The HEP portal – www.cthep.com – has been updated with your individual 2014 compliance status, so log-in soon and have your family members do the same. In addition, an age-based chart detailing the preventive HEP requirements is attached to this email.
All requirements must be completed between now and December 31, 2014, so please schedule any outstanding screenings as soon as possible. To receive the financial advantages of HEP, please remember that you and your dependents must have been compliant with the 2013 requirements.
Chronic Conditions
In additionto the HEP preventive requirements, all participants who have been diagnosed with one or more of the HEP chronic conditions must complete one of three HEP education and counseling requirement(s) to be compliant:
- Complete an interactive on-line survey or
- Read the educational materials or
- Speak to a CMS nurse
Both the survey and the educational materials are available on the 2014 HEP portal – www.cthep.com - and can be easily completed and submitted on-line. Please remember that participants are also required to accept or return phone calls from CMS nurses during the year if they are called.
The portal will also indicate when you complete your chronic condition counseling and education requirements.
Again, visit the HEP portal – www.cthep.com - to review your HEP status and schedule any required preventive appointments as early as possible. The portal also contains important tools to help you maintain your health, as well as information about 2014 HEP benefits.
