  • Important At-Home COVID Test Kit Update

    The Office of the State Comptroller announced how to order free at-home COVID-19 rapid test kits at in-network pharmacies through your state health plan coverage and how to pick them up at your local CVS location.

    How it works:

    • You will need to sign-on to your CVS account, or register for one.
    • Visit cvs.com or use the CVS Pharmacy mobile app. Click/select “At-Home COVID-19 Tests.”
    • Select "Request at-home tests" and answer the Medicare/Medicaid prompt.
    • Determine how many tests you need and enter your zip code to see nearby CVS locations with test kits in stock.
    • Select test kits and enter the information from your medical/pharmacy (Anthem/CVS Caremark) ID card.

    Within one hour, your test kits will be bagged and waiting for you at the front of the store (test kits cannot be retrieved at the drive-thru).

    Who is eligible to purchase test kits?
    Every plan member (including dependents) is eligible for no upfront out-of-pocket cost COVID-19 at-home test kits using their medical/pharmacy ID card (Anthem/CVS Caremark).

    Where can I purchase test kits?
    Members can purchase test kits at any retail pharmacy or merchant of their choice. However, please be aware that the plan will not reimburse out-of-pocket costs above $12 per test. For example, if you purchased a two-pack of test kits for $30, the plan would reimburse you $24.

    How many can I purchase?
    There is a limit of eight at-home tests per 30 consecutive days, per covered member.

    Can I purchase test kits online?
    You can purchase at-home tests online. However, you may be asked to pay out-of-pocket and will then have to submit for reimbursement. Any shipping fees are not covered by your pharmacy benefit.

    What if I paid out-of-pocket for test kits?
    Reimbursement for up to $12 per test is available with purchase receipt dated January 15, 2022 or later. See below for reimbursement instructions.

    Retail Purchases*

    1. Bring your test kit to the pharmacy counter. Be sure to present your medical/pharmacy ID card (Anthem/CVS Caremark) to the pharmacist.
    2. If unable to check out at the pharmacy counter, follow store instructions. Signage may direct you to pay for the over-the-counter kits at the front of the store.
    3. If you pay upfront for your kits, be sure to retain your receipts to apply for reimbursement (instructions below).

    *Members can go to any pharmacy and submit reimbursement for out-of-pocket costs. However, the maximum reimbursement will be $12 per test.

    Claim Reimbursement Process

    1. Go to CVS Caremark's reimbursement site here: COVID-19OTC 
    2. On the top right, sign-in or register to Caremark. (Required to request reimbursement)
    3. Upload your proof of purchase; this can be a digital receipt or photo of a paper receipt.
