    During Open Enrollment, which runs now through June 5, you may change medical and/or dental plans, add or drop coverage for your eligible family members, or enroll if you previously waived coverage.  Any changes you make will be effective July 1, 2015 through June 30, 2016 unless you have a qualifying status change. You may also enroll in the Health Enhancement Program (HEP), if you have not done so already. 

    At the Open Enrollment fair there will be representatives form the health, dental and pharmacy carriers to answer your benefit questions. 

    Also available onsite will be representatives from the following:

    •   Health Enhancement Program (HEP) to answer general questions about the HEP requirements for 2015.
    •   Prudential Retirement, the state's new third-party administrator (effective 7/1/15) of the Alternate Retirement Plan (ARP), the 457 Deferred Compensation Plan, and the 403(b) plan
    •  Employee Assistance Program (EAP) which provides brief, free, professional consultation and counseling, and workshops
    •  CTrides a program from the Department of Transportation which provides commuter services and options

    Enrollment Change Forms

    Employees interested in making changes may request enrollment change forms from the Benefits Unit at benefits@uconn.edu or (860) 486-0400.   As a reminder, any employee adding a dependent to his or her insurance is required to provide proof the relationships, such as a marriage license, or a full length birth certificate.

    Enrollment Deadline is June 5, 2015

    Completed forms with supporting documentation must be received in Human Resources by June 5, 2015.

    Notices for the open enrollment period have been mailed to your home.  The 2015 Health Planner, as well as a list of the Open Enrollment fairs around the state can be found on the OSC website at: http://www.osc.ct.gov/benefits/openenroll.htm
