  • CSD Exam Administration Process Update

    Due to technical difficulties, the Center for Students with Disabilities (CSD) online Exam Administration Form has been removed from the CSD website. Beginning Monday October 6, 2014 through the remainder of this semester, December 13, 2014, students will be required to complete a paper-based Exam Administration Form for all remaining exams or quizzes for the semester that will be proctored at the CSD and have not already been scheduled. This only applies to those exams or quizzes that will be proctored at the CSD.

    Students must complete the top section of the Exam Administration Form and work with their instructors to complete Section 2. Faculty must sign the completed form prior to students submitting the form in person to the CSD.  As always, completed forms must be submitted to the CSD at least 3 business days in advance of the quiz or exam in order to ensure sufficient time to provide accommodations.  Failure to provide adequate notice and/or incomplete forms may result in the accommodations not being available.
