  • CSD's Testing Administration Process has Changed

    The Center for Students with Disabilities (CSD) testing administration process has changed.

    For students who are approved for exam/quiz accommodations and their faculty cannot administer the exam/quiz either in class or within the department with the approved accommodations, exams/quizzes may be administered by the CSD.

    NEW Testing Administration Procedures:

    1. Students should discuss their approved exam accommodations with their faculty to determine where exams will be administered with the approved accommodations (e.g., in class, instructor’s office, at the CSD).
    2. Students should complete an online Exam Administration Form* for each quiz or exam to be administered and proctored by the CSD. The online form is available on the CSD website at http://csdwp.uconn.edu/exam-administration-form/.
    3. Once the online Exam Administration Form is submitted, the student will receive a confirmation e-mail, and the faculty will receive an e-mail requesting information regarding the administration of the exam/quiz from the CSD exam team.
    4. Faculty should respond to the CSD exam team e-mail in a timely manner to confirm and provide information regarding the exam/quiz in order for the assessment to be administered and proctored at the CSD.
    5. Students will receive a follow up e-mail from the CSD exam team confirming the arrangements and scheduling of the exam/quiz once the details have been confirmed with the faculty.
    6. Faculty should deliver the exam/quiz to the CSD in a timely manner via e-mail to csdexams@uconn.edu or confidential fax to (860) 486-4412. Exams may also be delivered in a sealed envelope by the faculty member, department designee (e.g., TA) or student. Faculty members should not send exams to the CSD through campus mail.
    7. Faculty should assist the CSD in making appropriate arrangements for exam/quiz return. Whenever possible, the CSD will return exams/quizzes via e-mail or fax. Faculty members or department designees may also pick up exams at the CSD. Faculty members may also request that the student return the exam in a sealed, security envelope. The CSD will not return exams via campus mail.

    *ONLINE FORMS MUST BE SUBMITTED TO THE CSD AT LEAST 3 DAYS IN ADVANCE OF THE QUIZ OR EXAM IN ORDER TO ENSURE SUFFICIENT TIME TO PROVIDE ACCOMMODATIONS. FAILURE TO PROVIDE ADEQUATE NOTICE AND/OR INCOMPLETE FORMS MAY RESULT IN THE ACCOMMODATIONS NOT BEING AVAILABLE.

    The CSD will continue to ensure the security and integrity of all exams/quizzes administered at the CSD.
