Anyone willing to donate please complete the UHP Sick Bank Form by downloading from the link, be sure to sign it, and fax it to Joann Lamere of HR at 860.679.4660 or to Mary McCuin at 860.674.0525.
Please know that confidentiality and dignity are protected and the recipients are not notified by HR or UHP of the names of the donors. So, for the members on the list, please know that they send you their sincere thanks for your kindness and generosity.
