  • CSD Exam Admin Forms for Finals Due

    If you are a student registered with the Center for Students with Disabilities (CSD) and are approved for exam accommodations, you are encouraged to speak to your instructors regarding your final exams as soon as possible.  If your professor(s) cannot provide you with the accommodation(s) for which you are approved by the CSD, please turn in an Exam Administration Form to sign up for each final exam as soon as possible, but no later than three business days before the exam:

    PLEASE NOTE:

    • Exam Administration Forms are available for download from the CSD website at http://csd.uconn.edu/forms.html or in the CSD office in Wilbur Cross Room 204.
    • The Storrs Campus final exam schedule is posted online at: http://registrar.uconn.edu/exams/
    • If you are approved for extended time and you have exams that are back-to-back, please discuss this with your instructors and the CSD to make appropriate scheduling arrangements.
    • All final exams accommodated by the CSD will be administered in the Wilbur Cross Building, North Reading Room. Check in for exams will be held in Room 110 of Wilbur Cross. The CSD will be open during the evenings of final exam week in order to accommodate 6:00pm exams. The CSD will also be open on Saturday, Dec. 13 and Sunday, Dec 14 for exams scheduled on those days.
    • The CSD has earplugs and noise-cancelling headphones available for use during exams.  Please feel free to ask for them when you check in for your exam.
    • To ensure the academic integrity of exams, the CSD will be using live feeds from video surveillance cameras to proctor exams.  No video will be recorded or stored.  If you are uncomfortable with the use of video cameras during your exam, a CSD employee can be arranged to proctor from within the room.  Please let us know when you submit your Exam Administration Form.
    • If you are having difficulty contacting your instructor to complete the Exam Administration Form and you do not think you will be able to be in contact quickly enough to hand in your completed form to the CSD three business days ahead of the exam, please contact csdexams@uconn.edu or call 486-2020 to discuss options as soon as possible.
