The HEP portal – www.cthep.com – has been updated with requirements due in 2016.
We encourage you and any family members to visit the portal early to find out what requirements you need to complete before the end of this year, by Dec. 31, 2016.
Schedule and complete appointments as soon as possible to avoid the late-year rush!
The portal – at www.cthep.com – also allows those with a chronic condition(s) to complete those requirements quickly and easily online.
