    Target Audience:  Physicians, Pharmacists, Students, Residents, and other health care professionals with needs, interestsor training in Health Information Technology and Health Information Exchange

    This is a knowledge-based activity.

    Learning Objectives:  Participants will (be able to):

    1.   Define models of Health Information Exchange used in Connecticut, Maine, and Rhode Island.

    2.   Identify the major healthcare delivery challenges that HIEs solve.

    3.   Describe how HIEs are addressing current and future COVID-19 health data needs

    Accreditation: The University Of Connecticut School Of Medicine is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.  The University Of Connecticut School Of Medicine designates this live CME activity for a maximum of 1 AMA PRA Category 1 Credit(s) ™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.

    The University of Connecticut, School of Pharmacy, is accredited by the Accreditation Council for Pharmacy Education as a Provider of Continuing Pharmacy Education and will award 0.10 CEU’s (1 contact hours) to pharmacists in attendance who complete an evaluation form; ACPE 0009-9999-20-051-L04-P CE Statements of Credit will be electronically uploaded to the CPE Monitor system within 24 hours of submission via the learning management system.

    Conflict of Interest Policy: All faculty members participating in CME activities provided by the University Of Connecticut School Of Medicine are required to disclose to the program audience any actual or apparent conflict of interest related to the content of their presentations.  All faculty members participating in CPE activities provided by the University Of Connecticut School of Pharmacy are required to disclose to the program audience any actual or apparent conflict of interest related to the content of their presentations. 

    Program planners have an obligation to resolve any actual conflicts of interest and share with the audience any safeguards put in place to prevent commercial bias from influencing the content. Neither the activity director, planning committee members, nor the speakers, Mr. Alfreds, Mr. Hackney and Dr. Sarkar, have a financial interest/arrangement or affiliation with any organizations that could be perceived as a real or apparent conflict of interest in the context of the subject of this presentation.

    Dr. Jeffery has a financial interest/arrangement with CVS Health that could be perceived as a real or apparent conflict of interest in the context of his role as an activity planner. The following safeguard(s) have been put in place to avoid the insertion of commercial bias into the content: The activity director will restrict the planner’s involvement to areas outside the area of conflict.

    Mr. Alfreds, Mr. Hackney, Dr. Sarkar and Dr. Agresta will not be discussing the off-labeled use of any product. This CME and CPE activity has no commercial support associated with it.
